Removing a Low Ceiling?

I own a 1 story bungalow with 7 foot ceilings and a hip roof. I don't need the attic, and the low ceilings are annoying, so I'd like to remove the ceilings, insulate the roof, and put drywall on the rafters. There are several issues involved that I can handle, one of them being the fact that I need to put 8 1/2 inches of insulation on 2 x 6 rafters (I'll tack 2 x 4s onto the rafters to extend them).
The first problem is roof ventilation. I can vent the top of the roof easily, but the ends the rafters rest on the top plate of the exterior walls, so the base of the roof is "sealed". Short of installing small vents at the base of the roof between each rafter, how can I properly vent the roof so I can put drywall on the rafters?
The second problem concerns windows. I want to install new windows as close as possible to the top of the 7 foot walls, but since I have to use 2 x 10 headers (I bought a 6 foot wide window), it ends up too low on the wall for my liking. The windows in place now are installed as high as possible (which I like), and have no headers above them - they have them underneath. Isn't that against code? If I install the new window that way, will the wall cave in on the window? What is the best way to install the window as close as possible to the top of the wall?
